Thank you for submitting a request for an autism evaluation. A member of our Team will contact you within 48 business hours to begin the evaluation process. In the meantime, to expedite the evaluation process, please see a summary of the documents that we will need to begin the evaluation process:

1. A photo of the front and back of your insurance card
2. A prescription from your primary care doctor for an autism evaluation. You should be able to call your doctor and request they send this directly to Lighthouse Autism Center. Our fax number is: 574-217-4894
