Lighthouse Autism Center Privacy Policy

LAC Intermediate, LLC DBA: Lighthouse Autism Center (“LAC,” “we,” “us,” or “our”) respects the privacy of every individual who visits its corporate website and uses the related services (collectively “Corporate Site”). This policy (“Privacy Policy”) provides notice of our information collection practices and of the ways in which your information may be used and disclosed with respect to the Corporate Site. This Privacy Policy may change from time to time, so please check back periodically to review this information. By using the Corporate Site, you consent to this Privacy Policy.  If you do not agree with the terms of this Privacy Policy, please discontinue use. 

This Privacy Policy does not apply when you access therapy services that are provided by our affiliated clinical practices, VAV Operations IL, LLC; VAV Operations MI, LLC; VAV Operations, LLC; VAV Operations IA, LLC; VAV Operations NE, LLC (“LAC Therapy Professionals”) or when we process your protected health information (“PHI”) on behalf of LAC Therapy Professionals as a “business associate.” For more information about how your PHI is collected, use, and disclosed, please refer to the LAC Therapy Professionals’ notice of privacy practices. 

 

Information Collected:

We collect the below categories of personal information from you when you access the Corporate Site. We, and our third-party vendors that support the Corporate Site, collect this information directly from you and automatically from your device or web browser when you visit the Corporate Site.  

  • Contact Information: Name, email address, postal address, and phone number.  
  • Website Activity: We collect information related to your access to and use of our Corporate Site, including the type of browser you use, access times, pages viewed, your IP address, and the page you viewed before and after visiting our Corporate Site. We may also collect this type of information in visual form when you visit our Corporate Site, including recordings and video snippets of your Corporate Site activity. 
  • Device Information: We collect information about the computer or mobile device you use to access our Corporate Site, including the hardware model, operating system and version, unique device identifiers, and mobile network information. 
  • Geolocation Information: We may collect the approximate location of your device from your IP address. Additionally, we may collect your precise geolocation with your permission. We may also collect information from your device in accordance with your device settings. 
  • Commercial Information: We collect information about your product and service preferences and interests. 
  • Social Media Information: Account names, handles, biographies, and other details that you make available on social media networks. 
  • Other information that you may provide to us or that we may infer based on the above.

We collect personal information directly from you in the below contexts. We also collect personal data from you using sources like advertising or marketing networks, data analytics providers, social media networks, and other service providers or third parties: 

  • If you access the Corporate Site 
  • In connection with your interactions, inquiries, or other requests related to the Corporate Site, such as if you fill out our “Contact Us” form 
  • If you share content linking to any of our accounts on social media platforms or interact with us via social media 
  • If you participate in a contest, promotion, or survey 
  • If you contact us via e-mail or through our customer service, including through the use of online chat tools that we may make available

We may use your personal information for the following purposes: 

  • To provide, operate, maintain, and protect our Corporate Site;

Disclosure of Information:

We are committed to maintaining your trust, and we want you to understand when and with whom we may disclose the information we collect. 

  • SMS Content/Communications:  The Company does not sell, rent, or lease its customer lists to third parties. SMS OPT-IN OR PHONE NUMBERS for the purpose of SMS are not being shared.  
  • Authorized third-party vendors and service providers: We may share your information with third-party vendors and service providers that help us with specialized services, including billing, payment processing, customer service, email deployment, business analytics, marketing, advertising, performance monitoring, hosting, and data processing. 
  • Social media platforms: Where you choose to interact with us through social media, your interaction with these platforms typically allows the social media company to collect some information about you through cookies and other digital tracking mechanisms that they place on your device. In some cases, the social media company may recognize you through these technologies even when you do not interact with their platform or may notify your connections about your interactions with us. Please visit the social media companies’ respective privacy policies to better understand their data collection practices and the controls they make available to you. 
  • Companies involved in advertising: We work with companies that assist us in advertising our Corporate Site to you and others who may be interested in the Corporate Site. These companies may use tracking technologies on our Corporate Site to collect or receive information over time and across different websites or platforms, including this Corporate Site and elsewhere on the internet, and use that information to provide measurement services and provide you with targeted ads. 
  • Corporate affiliates: We may disclose your information to subsidiary or parent companies of LAC. 
  • Business partners: We may work with other companies to provide you with certain product or service offerings. 
  • Business transfers: We may share your information in connection with a substantial corporate transaction, such as the sale of a website, a merger, consolidation, asset sale, bankruptcy, or similar business transaction or proceeding. 
  • Legal purposes: We may disclose information to respond to subpoenas, court orders, legal process, law enforcement requests, legal claims, or government inquiries, and to protect, defend, and investigate the rights, interests, health, safety, and security of LAC, our affiliates, users, or the public. 

 

Use of Cookies and Other Tracking Technologies:

We and our third-party vendors use cookies, pixel tags, and other tracking technologies on the Corporate Site in a variety of ways to enhance or personalize your online browsing experience. Cookies are small text files with small pieces of data placed on your computer’s hard drive by your web browser that enables us to personalize your website browsing session. These tracking technologies help us better understand your needs and preferences and tell us which parts of our website you have visited, facilitate and measure the effectiveness of our advertisements, and provide consistent and personalized services and experiences. These tracking technologies gather information over time and across different websites about you, some of which may be personal information as identified above in “Information Collected”. 

You can set your browser not to accept cookies or to notify you when you are sent a cookie, giving you the opportunity to decide whether or not to accept it. If you do not accept cookies, however, you may not be able to access utilize certain functionalities on our Corporate Site. Please note that our Corporate Site does not currently recognize “Do Not Track” signals. However, our Corporate Site may recognize certain opt-out preference signals, such as the Global Privacy Control, which we process as a request to “opt out of targeted advertising” and “do not sell” request, as applicable and in accordance with applicable state law. Choices Regarding Communications You Receive From Us:

LAC gives you choices regarding the collection and use of your information. If you have registered to receive LAC updates through our Corporate Site, and do not wish to be contacted, please contact us. Or, when applicable, please exercise your choice by opting out of any related email communications from LAC by using the “unsubscribe” feature included in the email. Please note that we may still send you messages in response to your inquiries or about other non-promotional items, such as legal notices or communications regarding the management of your relationship with us.  

 

Your Privacy Rights:

You may have the rights listed below (“Data Subject Rights”) with respect to the personal information that we collect or process about you. However, these rights differ depending on your place of residency, including Indiana, Iowa, Nebraska, Nevada, and Texas. Please note that we reserve the right to honor your Data Subject Rights to the extent required by applicable law. 

  • Right to Confirm Processing, Access, and/or Obtain a Copy: If you ask us, we will confirm whether we are processing your personal information. Additionally, upon request, we will provide you with a copy of all personal information you are lawfully entitled to receive, potentially including specific pieces of information, along with certain other details. 
  • Right to Correct: If you believe your personal information is inaccurate or incomplete, you may request that we correct it. 
  • Right to Delete: You may request that we delete personal information that we maintain about you.  
  • Right to Revoke Consent and/or Opt Out of Certain Processing Activities: You may ask us to restrict or stop certain processing of your personal information in specific contexts, such as if we process personal information that is considered “sensitive” under applicable U.S. state laws or engage in certain automated decision-making activities. 
  • Right to Opt Out of Targeted Advertising: Targeted advertising is the practice of serving you tailored advertisements based your personal information gathered over time and across other businesses, websites, applications, or services. You have the right to opt out of this practice. 
  • Right to Opt Out of Sales: Some states may consider targeted advertising a “sale” of personal information. You may request that we not “sell” your personal information. 
  • Right to Non-Discrimination: We will not discriminate against you for exercising Data Subject Rights, but we may charge a reasonable fee as permitted by law in fulfilling these rights, such as if you request multiple copies of your personal information. 
  • Right to Appeal: If we deny your request to exercise a Data Subject Right, you may have the right to appeal the decision with us. If you would like to appeal a prior decision, please be sure to include information about your prior request so that we may locate our earlier determination.  
  • Right to Lodge a Complaint: You may submit a complaint to the competent supervisory authority in the place in which you live if you have any concerns about our processing of your personal information. This may include your state’s attorney general if we deny your request to appeal our prior decision regarding your Data Subject Right.

In order to process your request to exercise a Data Subject Right, we will ask you to verify your identity by confirming your name, e-mail address, phone number, or other identifiable information that we have in our records, such as most recent interaction with us, if applicable. 

 

Data Integrity and Security:

While perfect information security is never possible, the Company has implemented commercially reasonable technical, physical, and administrative safeguards to protect your information against loss, misuse, alteration, or destruction.

 

Data Retention:

We retain personal information for as long as needed or permitted in light of the purpose(s) for which it was obtained and consistent with applicable law. The criteria used to determine our retention periods include:   

  • The length of time we have an ongoing relationship with you and provide the Corporate Site to you; 
  • Whether there is a legal obligation to which we are subject (for example, where we are required to maintain certain business records that may include personal information that relates to you); or

Children:

LAC does not knowingly collect, either online or offline, personal information from persons under the age of thirteen. If you are under eighteen, you may use the Corporate Site only with the appropriate consent of a parent or guardian.

If we decide to change our Privacy Policy, we will post those changes in this Privacy Policy, on the home page, and in other places we deem appropriate so that you are aware of our information practices. We reserve the right to modify this Privacy Policy at any time, so please review it frequently. If we make material changes to this policy, we will notify you here. Your use of the Corporate Site following any such updates will constitute your acceptance of such updates.
